During a busy night shift a well-staffed unit suddenly loses power to one bedside (CPAP/ventilator alarm) while the infant is desaturating and bradycardic. Describe your immediate actions, how you would stabilise the infant, how you would allocate team roles, and what escalation or documentation you would perform afterward.

I would move straight to hands-on stabilisation. If the ventilator/CPAP fails and the infant is desaturating to 75% with HR around 80, I’d grab the closest T-piece/bag and give effective ventilation with 100% oxygen while ensuring chest rise and tube/ prong position. I’d call for immediate help — one nurse to bag and monitor HR/SpO2, one to check the ventilator, switch to backup power or an alternative ventilator and prepare emergency drugs, and a senior nurse or registrar to lead and communicate with the family. My targets would be HR >100 and SpO2 >88% within 2 minutes. Once stable I’d document exact times, device alarms, actions taken and contact biomedical engineering and the consultant, file an incident report (Datix) within 24 hours and arrange equipment review to prevent recurrence.
